Blaming the player, not the game
When something goes wrong, the first question is almost always: who? Who made the mistake? Who dropped the ball? Who’s responsible? It feels like common sense. It feels like accountability. And it’s usually the wrong question.
System blindness is the inability to see that outcomes are produced by structures, not just by individuals. When a hospital has a high error rate, the system-blind response is to find and punish the doctors who made mistakes. The system-seeing response is to ask what about the shift patterns, the handover processes, the workload, and the interface design made errors likely - regardless of who was holding the scalpel.
This isn’t about letting people off the hook. Individual responsibility is real. But in most complex systems, individual behaviour is heavily shaped by the structure people operate within. The same person, placed in a different structure, produces different outcomes. Replace the person and the same structure produces the same problems with someone new. If the problem survives the replacement of every individual, the problem isn’t the individuals. It’s the system.
IN PRACTICE
The person-shaped hole
A school has high teacher turnover. The system-blind diagnosis: “We keep hiring bad teachers” or “People these days don’t commit.” The system-seeing diagnosis: the workload is unsustainable, the admin burden leaves no time for the work that attracted people to teaching, the pay doesn’t cover the cost of living in the area, and the performance management system punishes risk-taking. Fix the hiring all you want. The structure will chew through the new hires just like it chewed through the old ones.
A product team keeps shipping features late. The manager blames the engineers: not focused enough, too many distractions, poor estimation skills. But every team in the company ships late. The engineers are different. The managers are different. The projects are different. The lateness is the same. Something structural is producing the pattern - maybe the planning process, maybe the approval chain, maybe the culture of adding scope mid-sprint. Blaming the engineers is easier. Seeing the system is more useful.
A person keeps ending up in the same kind of difficult relationship. Friends say: “You pick the wrong people.” Maybe. But if the pattern repeats across very different partners, the system worth examining isn’t the partner selection. It’s the dynamic - the roles that get established early, the communication patterns that develop, the boundaries that do or don’t get set. The “type of person” you’re attracted to is often a proxy for the type of system you unconsciously build together.
WORKING WITH THIS
Seeing the water
The first move is suspicion. When you find yourself reaching for a person-shaped explanation - laziness, incompetence, bad attitude, wrong hire - pause. Ask: if I replaced this person, would the problem go away? If the answer is “probably not” or “it would just show up with someone else,” you’re looking at a system problem, not a people problem.
The second move is structural curiosity. Instead of “why did they do that?” ask “what about the situation made that the likely thing to do?” People respond to incentives, information, time pressure, and norms. Change any of those and you change the behaviour - without changing the person.
The hardest part is that system blindness feels righteous. Blaming people is satisfying. It gives us a villain and a story. Seeing structures is unsatisfying. Nobody wants to hear “the incentive design is suboptimal.” But the satisfying story produces fire-and-rehire cycles. The boring structural diagnosis produces lasting change. Systems thinking isn’t about being soft on individuals. It’s about being smart enough to see what’s producing their behaviour.

When this is in play
You’re seeing system blindness when the same problem recurs with different people and nobody asks why. When the response to every failure is to find someone to blame. When “accountability” means punishment rather than understanding. When a new leader is brought in to “fix the culture” and leaves two years later having encountered the same resistance as their predecessor. When the phrase “if only we had better people” is the diagnosis - that’s the system blaming its components for its own design.
